I work in a public hospital in the North West Province in South Africa. I have to add that this issue is also worsened by factors such as language barriers, especially in our country where some nurses struggle to understand English during report handover with shift changes. A major problem as well is that due to our large wards, up to 50 patients per ward, the nurses "rush" through report handover, just to get it finished, and then sometimes leave out some important detail. I think that this is a problem that will be difficult to address, due to the lack of resources of creating more wards, with less patients, with adequate amount of staff, as there are no sufficient funding for this purpose and due to our severe shortage of nurses. I feel this is a major contribution to the lack of optimal and qualitative patient care.
